Columbus Camera by Blair Camera Co.

The Columbus Camera was in manufactured from late 1891 and named in honor of Chicago's Columbian Exposition. It was a 4x5 box camera For roll film taking 25 to 100 exposures. The camera was similar to the hawk-eye '95 box camera.
